Filing Clean Hands Form

The Department of Insurance, Securities and Banking has changed the way you submit your Clean Hands Form to the District of Columbia.

• Go to DISB.dc.gov
• Click on the links: Regulated Entities > Securities > Licensing Requirements
• Scroll to the bottom of the page and click Clean Hands
  • It will take you to the website “MyTax.DC.gov” where you will see options for two categories -- select either:
    • Businesses 
    • Individuals

Note: You must register with Office of Tax & Revenue (OTR) to use this site, so create your username and password.

After you have registered, scroll to the bottom and click on the category in which you are filing.
