We are filing new rate tables for Policy Forms 97058 and Rider Forms 99572, 99573, and 99574. This filing represents an average increase of 9%. Rate increases will range from 2% to 10% on forms 97058, 99572, and 99573. There will be a rate decrease on form 99574. The rate for form 99574 will go from 30% to 22% of premium. These new rates will apply on the policyholder’s next renewal. These rates will be implemented approximately 135 days after approval.
