Effective immediately, companies submitting individual and group accident and health forms to the Department of Insurance, Securities and Banking are required to comply with the Uniform Child Health Screening Requirements and Reporting Form Act of 2004, effective April 13, 2005 (D C Law 15-353; D.C.. Official Code § 7-761.01 et sect)
