The Guardian Life Insurance Company of America - Rate Filing

These forms are new and do not replace any other forms. These forms are part of a new group policy and certificate series which has been developed in a new format and style in order to provide our policyholders with more simplified, less redundant contract forms. 
Policy forms GP-1-AH-11-DC, et al. reflect Guardian's new policy format, a shell form setting forth the terms and conditions between the policyholder and Guardian. These forms were submitted in SERFF under GARD-127093283 and Filed for Information with your department on 6/6/11. Certificate forms GC-STD-11-DC, et al. describe the disability insurance provisions applicable to an insured person and will be incorporated into the policy as an integral part. Certificate forms are attached for review.
