The attached rates are being submitted for approval. This is a new filing and will not replace any other rates on file with your department. Please note that these are custom rates/benefits for one group only and will not be used for other groups.
The Master policy and Certificate to accompany these rates were approved by your department on 2/28/2012. These will be marketed on a voluntary group basis to the employees of the employer, through individual solicitation. The plan provides benefits for the employee for disability resulting from both an accident or a sickness.
