The attached rates are being submitted for approval. This is a new filing and will not replace any other forms on file with your department. These documents are in final form. Please note that these are custom benefits/rates for one group only and will not be used for other groups.

The Master Policy and certificate to accompany these rates were approved by your department on February 28, 2012. This plan will be marketed on a voluntary group basis to the employees of the employer, through individual solicitation. This is an Accidental Injury Policy with provides benefits for sickness and dependent coverage. The coverage provides hospital benefits, disability benefits and accidental injury benefits for the employee both on and off the job, when injured in a covered accident.
