Berkley Life and Health Insurance Company - Rate Filing

Berkley Life and Health Insurance Company wishes to submit the enclosed Blanket Accident and Sickness insurance actuarial information for review and approval. The forms are new and are not intended to replace any existing forms previously filed and approved by Department.
Upon approval by the Department, the marketing will be through licensed producers. All marketing methods will always be employed in accordance with state law and regulation.
This is a blanket student accident and sickness program which provides coverage to college students for certain expenses incurred from injury or sicknesses while enrolled in college. Upon approval the Policy will be issued to colleges on a blanket basis and will be available to eligible students. Spouse and dependent coverage will also be available for additional premiums. This is a limited duration, nonrenewable policy.
