Form 11120 is an individual health policy that provides listed benefits which are a result of an accident. The benefit amounts are at the election of the proposed insured. The range of benefits is shown on page 3 under POLICY SCHEDULE OF BENEFITS AND PREMIUMS. Each policy is guaranteed renewable to age 65 and premiums are subject to change, as described in the policy. The issue ages and annual premiums rates are shown in the attached actuarial memorandum.
