The following information is required to fulfill your request for a speaker. Be sure to allow 2 to 4 weeks advance notice, if possible:
- Name of organization
- Contact information
- Date and time of presentation (first and second choices)
- Address of facility where presentation will commence
- Estimated participants (minimum of 20 participants)
- Bilingual or hearing-impaired interpreter
- Topic of interest (see above)
